LIFEPlan CCO announces launch of improved care coordination program in New York State Individuals with disabilities are encouraged to enroll in a Care Coordination Organization (CCO) in their area to maintain eligibility for OPWDD (Office for People With Developmental Disabilities) Waiver Services by July 1, 2018 SYRACUSE, N.Y., June 27, 2018 – Seven approved Care Coordination Organizations (CCOs) will begin providing improved People First Care Coordination to residents of New York state with intellectual and developmental disabilities (I/DD). This will take place on July 1, 2018, under the New York State Department of Health’s Health Home Care Management program in partnership with the Office for People With Developmental Disabilities (OPWDD). LIFEPlan CCO, one of the seven approved CCOs, is a partnership of over 72 local, nonprofit organizations serving 38 counties throughout the Central, Northern, Southern Tier, Hudson Valley and Capital regions of New York. The organization is expected to serve more than 17,000 people and to employ approximately 600 professional staff, including over 500 care managers. “This new system will revolutionize the way individuals with disabilities will receive care coordination services,” said Nick Cappoletti, CEO of LIFEPlan CCO. “Under CCOs, individuals will now receive one single comprehensive and holistic plan with a greater focus on outcomes, measuring the real, positive impact they have on individuals’ lives. Supports, services and even outside services will be combined for better visibility to the patient, family, caregiver and health care provider. The overall well-being of individuals with disabilities now holds significant weight in their care.” Cappoletti has a child with a disability, which gives him a unique perspective as the leader of a CCO. LIFEPlan stands out among other CCOs as the only state-approved CCO that is led by a parent of a child with a disability, and has several team members and agency partners who are also parents of individuals with disabilities. The organization plans to utilize the personal experiences of its leaders and staff members to provide services that focus on individuals and the vision of what each of its members want to accomplish in the future. According to the state, CCOs will be a family’s “single access point” for services. The enhanced services are coordinated by care managers, formerly titled Medicaid Service Coordinators, using new electronic records to create a single individualized plan, called a Life Plan, which will replace the current Individualized Service Plan (ISP). Individuals currently receiving Medicaid Service Coordination (MSC) or Plan of Care Support Services (PCSS) who want to participate in the new Health Home Care Management program must sign a form through their current service coordinator to enroll at a CCO of choice by July 1, the day Care Coordination begins.
